Collect sample 30 to 60 mins pre dose
Suggested Therapeutic Targets:
Empiric therapy: Trough 2-7 mg/L
Directed therapy: Trough 4 x MIC
Toxic Range: >7 mg/L
Measure trough levls at least 48-hours following initiation or dose change of linezolid
